To see if you are eligible to apply for one, please refer to this guide.  Please have the following documents and information ready when you come into our office:

• A completed Application Form for Certificate/Document (a hard copy is also available in our office if needed)
• An acceptable identification
  • Canadian Passport
  • Previous Alberta Birth Certification
  • Alberta Driver License / Identification Card
  • Government issued Photo Identification
